Eligibility: Sweepstakes (the "Promotion") is open only to those who sign up at the online sweepstakes page located at https://www.longevitywiz.com/sweeps/lw_scale and who are at least 18 years old at the time of entry. The Promotion is open to the legal residents of the US including the District of Columbia, Canada, South Africa, Australia and New Zealand. The Promotion is sponsored by Longevity Wiz, Universal Family Publishing, LLC. (the "Company”, “Companies”, “we”, “us”, “Longevity Wiz” or “Sponsor"). Employees of the Sponsor their respective affiliates, subsidiaries, advertising and promotion agencies, suppliers and their immediate family members and/or those living in the same household of each are not eligible to participate in the Promotion. The Promotion is subject to all applicable local laws and regulations and is otherwise void where prohibited.
2. Agreement to Rules: By participating, you agree to be fully unconditionally bound by these Rules, and you represent and warrant that you meet the eligibility requirements set forth herein. In addition, you agree to accept the decisions of Longevity Wiz as final and binding as it relates to the content. 3. Promotion Period: Entries will be accepted online starting on or about 10/30/2025 12:00:00 AM EDT and ending 02/18/2026 11:59:59 PM EST. All online entries must be received by 02/18/2026 11:59:59 PM EST. All times are (GMT-05:00) Eastern Time (US & Canada).
4. How to Enter: In order to enter the Promotion, you must complete the applicable Promotion entry form (“Promotion Entry Form”) on the website. The information requested may vary but must include at a minimum your valid email address. Once you submit the requested information on the Promotion Entry Form and click the ENTER NOW button or ENTER TO WIN! button, you will receive one (1) entry into the Promotion. The Promotion must be entered by submitting an entry using the online form provided on this site. The entry must fulfill all Promotion requirements, as specified, to be eligible to win a prize. Entries that are not complete or do not adhere to the rules or specifications may be disqualified at the sole discretion of Longevity Wiz. You may enter up to 3 times per day and you must fill in the information requested. Winner selection and notification: Within ten (10) business days of the conclusion of the Promotion Period, one (1) winner (“Winner”) will be selected in a random drawing under the supervision of the Sponsor from among all eligible entries received during the Promotion Period. Winners will be notified via e-mail to the e-mail address they entered the Promotion with within five (5) days following the winner selection (“Winner Notification”). Longevity Wiz shall have no liability for a winner's failure to receive notices due to winners' spam, junk e-mail or other security settings or for winners' provision of incorrect or otherwise non-functioning contact information. If the selected winner cannot be contacted, is ineligible, fails to claim the prize within 15 days from the time award notification was sent, or fails to timely return a completed and executed declaration and releases as required, prize may be forfeited and at Sponsor’s sole discretion an alternate winner may be selected. The receipt by winner of the prize offered in this Promotion is conditioned upon compliance with any and all federal and state laws and regulations. 6. Prize Description: The value of the prize is three hundred and fifty-two dollars ($352.00). The prize Winner will receive three hundred and fifty-two dollars ($352.00) by check or other form chosen by Sponsor in its sole discretion within thirty (30) days of their response to the Winner Notification. You are not guaranteed to win a prize and your chance of winning is dependent on the total number of eligible entries received. Actual/appraised value may differ at time of prize award. The specifics of the prize shall be solely determined by the Sponsor. No cash or other prize substitution permitted except at Sponsor's discretion. The prize is nontransferable. Any and all prize related expenses, including without limitation any and all federal, state, and local taxes shall be the sole responsibility of the winner. No substitution of prize or transfer/assignment of prize to others or request for the cash equivalent by winners is permitted. Acceptance of prize constitutes permission for Longevity Wiz to use winner's name, likeness, and entry for purposes of advertising and trade without further compensation, unless prohibited by law.
